How to request personal health information (medical records) from Community Memorial Hospital:

• Contact the Health Information Management (HIM) office at Community Memorial Hospital by calling 507-457-4476 (Monday through Friday 8 am to 4:30 pm).
  
  
• Download and complete an authorization form: Authorization for Disclosure of Medical Information.
  
  
• If you have questions regarding completion of the authorization form, please contact the HIM office and they will assist you.
  
  
• Please mail (855 Mankato Avenue, Winona, MN), fax (Health Information Management fax phone 507-457-4160) or deliver your form in person to HIM office. The form MUST be signed by patient or guardian before submitting.
  
  
• Please include your phone number with any request for medical records.
