Transitional CareLake Winona Manor opened a Transitional Care Unit in January, 2008 to provide post-hospital assistance to patients preparing to return to their homes.

"Although a patient may no longer require hospital care, it may be too challenging for them to make the adjustment from hospital to home," noted Mary Miller-Hyland, Lake Winona Manor (LWM) administrator. "Our Transitional Care Unit provides just the right level of care for these people."

Physicians work closely with patients and their families to determine if the Transitional Care Unit is an option. "In general, our transitional patients need a certain level of skilled care, such as rehabilitation services, physical or occupational therapy, or IV therapy," said Patti Volkman, director of nursing at Lake Winona Manor. "The average length of stay is two to four weeks-or whenever the skilled need is no longer appropriate."

Winona resident Jeanne Boller was admitted to LWM's Transitional Unit on January 30. On February 13, she was preparing to return home. "The care has been very good here. They take care of you as an individual-everyone has been just wonderful."

The Transitional Care Unit also provides an opportunity for people to receive additional education about managing a chronic disease. "When people have a better understanding of how to effectively manage their disease, they are less likely to require re-hospitalization," Miller-Hyland noted.

Lake Winona Manor has long provided short-term care for individuals who require rehabilitation services, but the Transitional Care Unit expands its ability to offer short-term services. When individuals are ready to return home, Winona Health also provides a broad range of Home Care services-from homemaker services that help with meal preparation and cleaning, to skilled nursing services that help individuals manage chronic conditions.

"We're committed to transforming community healthcare," said Miller-Hyland. "We're continuously looking ahead and looking at things in new ways to ensure we have the right services in place to meet our communities' needs.
