The Winona Health Community Care Network is a free service available to community members who would like help with managing a chronic health condition. Its goal is to support and improve the overall health of individuals who are at high risk for frequent emergency department visits and hospital admissions.

Individuals interested in becoming a Community Care Network client will meet with a Community Care Network team members who will assess their needs to determine whether this service will be beneficial. They also will help determine what other community resources may be helpful.

The Community Care Network:

  • provides support to individuals by bridging gaps in care
  • improves healthcare outcomes
  • improves an individual’s overall health
  • improves an individual’s quality of life
  • reduces Emergency Department visits
  • reduces hospital admissions
  • reduces an individual’s healthcare costs
  • reduces overall health care costs in our community

The Community Care Network team includes:

  • nurses
  • social workers
  • dietitians
  • physical therapists
  • student health coaches
