Community Health Needs Assessment

Responding to the health needs of the community, especially the most vulnerable among us, is core to achieving our mission of optimal health and well-being. To do so, we must have an understanding of these needs and develop a deliberate approach to addressing the needs. In addition, the 2010 Affordable Care Act requires that all 501(c)(3)hospitals conduct a community health needs assessment every 3 years.

The needs assessment process examines community demographics, socio-economic factors and health service utilization trends.  Input from community coalitions, focus groups, and surveys is incorporated into the report.  This assessment incorporates components of primary data collection and secondary data analysis that focus on the health and social needs of Winona County using the zip code 55987.
