Ucare Provider Award 2023 Winona Health WebWinona Health was recognized for quality of care at UCare’s 15th annual “A Salute to Excellence!” on September 21, 2023. UCare is a community-based, nonprofit health plan. The event recognized clinics and care systems with top results in 2022 Healthcare Effectiveness Data and Information Set (HEDIS) measures – including preventive care, disease management, behavioral health, maternity care, care transitions and medication management. Winona Health was honored for improving the health of UCare members enrolled in Medicare, Special Needs Basic Care (SNBC), and the Prepaid Medical Assistance Program and Minnesota Care (PMAP/MNCare). Winona Health was an early innovator in developing and evaluating methods to improve patient outcomes through transitioning to a value-based healthcare model. A value-based healthcare model demonstrates lower cost and higher quality care, rather than a traditional fee-for-service model. “Every person who comes to Winona Health is unique and their needs are unique as well. Our caregivers, whether they are behind the scenes or providing direct care, are focused on making sure each of them receives what they need,” said Merry Enright, quality coordinator at Winona Health. “That includes preventive care and screening, care for whatever concerns they may have and, when there are community resources that may be helpful, ensuring that they can get connected with those resources.” “At UCare, we are deeply grateful for the extraordinary efforts our health care partners take to put their patients and UCare members first,” said Marden-Resnik at the event. “This evening of celebration, gratitude and awards is one of the highlights of the year. We enjoy taking the time to recognize health care partners like you who enable us to fulfill our quality care promise to members.” To schedule an appointment with a primary care provider at Winona Health, call 507.454.3650.
