Winona Health recognized for quality performance

Winona Health was recognized for quality of care at UCare’s annual “A Salute to Excellence!” event in September.

UCare, a community-based, nonprofit health plan, recognizes clinics and care systems with top results in preventive care, disease management, behavioral health, maternity care, care transitions and medication management.

“All of us at Winona Health, working behind the scenes or providing direct patient care, are here to make sure each person receives the care and support they need,” said Merry Enright, quality coordinator at Winona Health. “That includes checkups and preventive care, health screenings, managing ongoing health conditions and navigating specialty care as needed. We also connect individuals and families with community resources to help with needs beyond healthcare.”

Kelly Fluharty, care transformation manager at Winona Health, said, “We are also collaborating with patients to continually improve care through our Patient Experience Advisory Committees. These are amazing learning spaces where patients share their thoughts and ideas to make their care and experience even better. Our goal is to provide the highest quality of care from all perspectives.”

UCare awards are based on Healthcare Effectiveness Data and Information Set (HEDIS) measures – including preventive care, disease management, behavioral health, maternity care, care transitions and medication management. Winona Health was recognized for improving the health of UCare members enrolled in the Prepaid Medical Assistance Program and Minnesota Care (PMAP/MNCare).

Winona Health was an early innovator in developing and evaluating methods to improve patient outcomes through transitioning to a value-based healthcare model. A value-based healthcare model demonstrates lower cost and higher quality care, rather than a traditional fee-for-service model.
