The Winona Health Pediatric providers are teaming up to offer sports physical appointments for middle and high school students on the second floor of the clinic at 855 Mankato Avenue in Winona, during the following times:
• Tuesday, August 5, 8 a.m. – 12 p.m.
During these times, sports physicals are available by appointment for $45, payable by cash or credit card at check-in. Insurance will not be billed.
Not sure when your child needs a sports physical? If your child is in 7th grade and wants to participate in school sports or some other after-school activities, they’ll need a sports physical. Minnesota requires these every three years starting in 7th grade. Some organizations in our community also require sports physicals as young as age 10 years of age. It can be included as part of their annual comprehensive well-child visit.
It’s helpful for parents to complete their portion of the sports physical form ahead of time. Minnesota and Wisconsin forms can be found online at www.winonahealth.org/sportsphysicals.
Parents of children who were previously diagnosed with heart conditions or head injuries must schedule an appointment with a member of their regular healthcare team.

Please note that clinic visits scheduled outside of our special sports physical days are not eligible for the $45 sports physical rate.
