Caregivers in Winona Health’s Pediatric and Adolescent Medicine department want to remind parents that if a child is due for an annual wellness visit, which is covered by most insurance, they can also include a sports physical at that appointment for no additional charge.

Sports Physical Dates

Winona Health Pediatric providers are teaming up to offer sports physicals by appointment for $45* for middle and high school student athletes on the second floor of the clinic at 855 Mankato Avenue in Winona, at the following dates and times:

Tuesday, July 29 • 1-4 p.m. Tuesday, August 5 • 8 a.m. to noon

The special $45 rate (payable at check-in – insurance will not be billed) is only for these dates. Those who prefer to schedule a clinic visit on a date that works best for them for a well-child check, sports physical, or both may call the Pediatrics department to schedule an appointment.

*Sports physicals requested outside of these dates are not $45 and co-pay/co-insurance/deductible may apply. If additional screenings or medical needs arise, co-pay/deductible may be applicable.

Sports physical forms

Students under 18 must be accompanied by a parent or guardian unless appropriate authorization form has been completed. Students previously diagnosed with heart conditions or head injuries must schedule an appointment with a member of their regular healthcare team.

Sports physicals requested at Urgent Care are not $45 and are not covered by insurance. Please use one of our other options for your sports physical. If additional screenings or medical needs arise, co-pay may be applicable.
