Need a physical?

Now is a great time to schedule your child’s camp, school or sports physical. Winona Health Pediatrics and Family Medicine departments remind parents that a well-child checkup is the perfect time to complete your child’s sports physical. Parents are encouraged to schedule sports physicals at the clinic with a Primary Care provider, and appointment times are available. Students under 18 must be accompanied by a parent or guardian.

“If your child is due for a well-child exam, it’s best to schedule a well-child appointment and bring your sports physical forms,” said Winona Health Pediatrician, Sarah Lallaman. “That way we can take care of everything for you in one visit.”

Students previously diagnosed with heart conditions or head injuries must schedule an appointment with a member of their regular healthcare team. To schedule an appointment or if you have questions, please call:

Please complete your portion of the form prior to your appointment. Forms are available for download below.

Sports physicals become part of the student’s Electronic Medical Record. Minnesota requires a sports physical every 3 years; Wisconsin every 2 years.
