Winona Health’s Weight and Wellness Program offers a thoughtful, holistic, whole-person approach to weight and wellness. That means we look at how nutrition, physical activity, behavior, and medical care all work together—and what that looks like in your daily life.

You’ll have a care team that focuses on what matters most to you—working with you to build a plan that feels realistic and sustainable.

Your care may include support with nutrition counseling, physical activity, behavior change, and, when appropriate, medical weight loss options—all designed to support long-term health.

Program eligibility

Ready for change You don’t need to have everything figured out. What matters most is a willingness to take a first step and keep going, even when it’s not perfect. We’ll support you through the ups and downs of your weight loss journey.

Connected to a Winona Health primary care provider Working with a Winona Health primary care provider helps keep your care coordinated—especially if you’re managing chronic conditions like diabetes, high blood pressure, or heart disease. If you don’t have a provider, we can help you get connected.

Age 18 and older The Weight and Wellness Program is designed for patients 18 and older.

Body Mass Index (BMI) over 35 Staying connected with your care team is an important part of your success. Regular visits help you track progress, talk through challenges, and adjust your plan as your needs change.

If something comes up, we understand. You can reschedule most Weight and Wellness Program appointments through your My Winona Health patient portal. If you’d rather talk with someone, we’re here to help.

Your care team

You’ll have a care team that works together to support you with a holistic, whole-person approach to your health and weight. Your plan is built around your needs, your goals, and your day-to-day life.

Here’s what you can expect along the way:

Your provider helps guide your overall care and keeps everything connected.
  • First visit: We’ll take time to understand your health history, past experiences with weight loss, and any factors that may be affecting your health. Lab work may be part of this visit.
  • Follow-up visits: We’ll check in on progress, talk through any challenges, and review medications if they’re part of your care plan.
  • Pharmacist support is also available to help answer questions about medications and navigate insurance coverage.
Behavior change is a meaningful part of plan. Your therapist will work with you to set realistic goals and build strategies that fit your life.
  • Focus: Identifying realistic goals, overcoming obstacles, and developing long-term strategies for weight maintenance.
  • Topics may include: Readiness for change, barriers to progress, coping strategies, managing expectations, and setting boundaries with friends and family.
Your dietitian provides nutrition counseling tailored to you. This includes understanding your current habits and helping you make practical, sustainable changes that support your health and weight loss goals.
Movement looks different for everyone. We’ll help you find a starting point that feels safe and manageable. Your exercise program begins in our Cardiac Rehabilitation Department, where you’ll be introduced to safe, foundational exercise and guided movement. From there, we’ll help you transition into a personalized plan that supports your lifestyle, goals, and long-term well-being.

Financial assistance & Patient Resource Center Located on the first floor of the clinic, the Patient Resource Center offers free, in-person support to help you better understand healthcare costs, insurance coverage, and financial assistance options.

Frequently Asked Questions

Body Mass Index (BMI) is a medical screening tool that measures the ratio of height to weight to estimate body fat. Higher BMI generally correlates with higher body fat. While BMI does not diagnose health, it helps assess health status and associated risks.
Higher BMI increases the risk of obesity, heart disease, high blood pressure, type 2 diabetes, gallstones, osteoarthritis, sleep apnea, certain cancers, depression, and other conditions.
Timelines vary by patient needs. A sample schedule may look something like this:

Not all patients require medication. This will be determined during provider visits and may depend on insurance coverage and other medications.
Once goals are met, you will transition to a long-term plan. Most patients remain in the program for 12–18 months, though transitions vary.
Your provider will discuss the referral process and help establish next steps if you choose this path.
